Transition Services

Our collaborations with Michigan Rehabilitation Services (MRS), Bureau of Services for Blind Persons (BSBP), the State of Michigan and Centers for Medicaid Services (CMS) allow us to serve as navigators as you transition to new chapters in life. This includes the transition from youth to adulthood as well as transitioning from a nursing facility to independent living.

 

Youth with disabilities between the ages of 14 and 26 who are in the transition process from school to work or community-based living receive transition services as they are preparing for postsecondary life.

 

Nursing Facility Transition Services are avilable for individuals residing in nursing facilities who have a desire to return to community based living.
