Pre-Employment Transition Services

DNEM provides a wide variety of pre-employment services through a partnership with Michigan Rehabilitation Services (MRS) that provide youths with the tools to ensure a successful transition into adulthood and independence.   Programs include but are not limited to:

Nursing Facility Transitions (NFT)

The Nursing Facility Transition (NFT) Program helps provide options for individuals to transition from a nursing facility back into the community. This service is available to all nursing home residents who are current or potential Medicaid recipients. NFT operates under a Person-Centered Planning philosophy, which means the transitioning individual will make all of the decisions, have their wishes acknowledged, and devise a plan that will fulfill as many of their life goals as possible.

School Transitions

Transition services facilitate and support the movement of high school students with disabilities into adult life. DNEM offers numerous services and opportunities to assist youth with disabilities transition into adulthood. We offer advocacy services for youth and families in creating their high school transition plans, as well as IEP and 504 meeting support. DNEM can provide independent living skills assessments for students that are looking to live on their own. We also provide a link to adult community services including, but not limited to, Social Security, health insurance options, housing, and workplace accommodations.
